eRegistration is now open for the Decorah Community School District

Walk-in registration for the 2018-2019 school year will be held at Decorah High School on Thursday, August 2nd, from 10:00 a.m. until 6:00 p.m., and on Friday, August 3rd, from 8:00 a.m. until 3:00 p.m. Parents/guardians are asked to complete eRegistration prior to walk-in registration. This will significantly reduce wait time during registration.

You may access eRegistration at https://decorah.powerschool.com Log in using your Powerschool login information. Parents/guardians must complete eRegistration for each student.

Tips for eRegistration:
Emergency contacts should not be the parents. DCSD will always attempt to contact the parents first in any emergency situation.

Addresses need to be spelled out in full (Avenue, Street, Drive, etc.). DCSD will be integrating the lunch system and Powerschool, so checking to be sure your address is identical for all family members will help with this process.

Permissions must be completed prior to students participating in practice, competetion, or extra-curricular activities. Student MacBook Airs and student iPads will NOT be issued until the Acceptable Use Policy (AUP) is confirmed.

Allergies and special health concerns need to be added on BOTH the Parent/Guardian and Health pages. This information eliminates the Health Form which was previously filled out by hand.
 
5th - 12th grade students must log into the STUDENT PowerSchool Portal to certify or provide assurance they understand certain items. This is also replacing paper copies of forms AND must be completed prior to students practicing, competeing, or participating in extra-curricular activities. Student MacBook Airs will NOT be issued until the AUP is confirmed.

From the School Nurse (Forms to turn in at registration):
• New 4yr olds - physical form and Immunization Record
• Kindergarten - updated Immunization Record and dental exam form, if not already on file
• 7th grade - proof of Tdap and Meningococcal vaccine, by August 24th
• 9th grade - proof of dental screening, valid up to 1 year prior to enrollment, by August 24th
• 12th grade - proof of two (2) doses of the Meningococcal vaccine, for students born after September 15th, 1999
