The Iowa Department of Public Health has notified the public that people who were born from May 1993 to October 2009 and were issued a pink and blue birth certificate 'wallet card' that measured 2 5/8" x 4" need to get a replacement.
Winneshiek County Recorder Teresa Bockman says her office will be able to help county residents with this process. Says Bockman, "You will still need to fill out the application and relinquish your 'wallet card' birth certificate, but we can notarize your signature and take a copy of your photo ID here. It should take us about 5-10 minutes to have the new certificate ready."

There is NO COST to you for this service, but you must present your old birth certificate to be replaced.
If you were born between 1993 and 2004 but were NOT born in Winneshiek County, you can still come to the Recorder's Office and staff members will help with the paperwork and send it to the state. The Recorder's Ofcice does not have out-of-county births for that time frame.
